Distal tibia malunion/distal fibula malunion repair ☠️ . . . Controversial case- check back to the previous case for injury details and discussion. This is a middle aged man with type 1 dm who sustained a low energy pilon, unknown chronicity. He controls his diabetes very well, but has a dense neuropathy. . . . I decided to fix him because 1 ) dm well controlled 2 ) he’s very responsible/compliant 3 ) normal vascular studies 4 ) high functioning. I think it’s worth giving the ankle a shot in a patient like this. . . . Given the unknown time course I wasn’t sure if his tibia was healed or not- plan was to either reduce and fix it or osteotomy and fix it. Main message here is have plan A and plan B (and be ready for plan c-d... ) After an anterolateral approach and lateral distraction, it was clear that this was a tibial malunion. . . . Being good doctors, we had done our trigonometry and were prepared. Best way to restore native anatomy (RONA ) is a supramalleolar opening wedge osteotomy at the Cora. Once the tibia was done, fibula needed to be dragged down to match. . . . In order for this to work, the lateral skin must be very supple (it’s going to get stretched ). Vascular studies must be normal (to heal surgical wounds ). DM must be well controlled. The articular surface needs to be in good shape. Patient must be complaint. Lastly- patient needs to know this might not work. . . . Technical details- 1 ) lateral distractor 2 ) k wires to guide osteotomy. 3 ) leave a medial hinge 4 ) structural graft + autograft, slightly over corrected 5 ) compress the shit out of the graft, get alignment perfect 6 ) locking plate, extra fixation. 7 ) restore fibular length, stable fixation. . . . #ortho #trauma #surgery #orthopaedics #orthopaedicsurgery #aotrauma #malunion #nonunion #deformity #correction #whatwouldyoudo #xray #premed #medschool #residency #fellowship #fixerupper #giveitashot #bestshot

✅Rotation #5 : Emergency Medicine 🚑 Wow! I went into this rotation thinking that the ER is super daunting and feeling under-qualified (comparing myself to half my class who have had emergency med experiences in their pre-PA past lives ). But this may actually have been my favorite rotation so far!! Highlights: -Working with preceptors who are uplifting and genuinely want you to learn and improve, and working with PAs! Most of my rotations so far have not included PAs. Don't get me wrong, I've worked with some amazing providers, but I am left guessing what a PA role looks like in x settings. -Being so busy that the days fly by! My last two rotations have been on the slower side due to Covid so the ER energy was actually refreshing lol. That being said, because this rotation was so hours heavy, I was starting to burn out this last week and have had little time to study for the EOR. One word: Podcasts. I recommend EMbasics and EMRAP. -Getting to see a wide variety of medicine and doing procedures! I liked that in the ER you can see a little bit of everything, and I always made it a point to see patients with lacerations or something that would potentially involve a procedure. Always happy to be able to practice my skills. -Not a "highlight" but, seeing our primary care gap in the face. It's a combination of many barriers like patient education (not everything is an emergency ), means and access (not everyone has or can see a pcp ), language barriers, Covid, lack of preventative medicine, and everything in between that gnaws me daily. Our system needs to do better. Finished up my last shift tonight and planning to spend tomorrow studying for the EOR exam. And then onto the next!

Implicit bias, microaggressions, allyship for LGBTQIA+ and advocating for social justice as a physician were topics on today’s agenda for day 4 of med school. So much great material to unpack and not enough time! 👏🏽 • I appreciated all of the panelists and folks involved in making today’s workshops possible because it is so important to have these difficult conversations. Sometimes it might be hard to discuss topics like sexism, racism or microaggressions, but the profession we are going into involves supporting human beings whose lives are shaped by these social intersectionalities. As physicians we cannot avoid these conversations because they are uncomfortable. Instead, we must embrace them and work towards learning about topics that we may not be as familiar with. Just like spending time studying for immunology or pharmacology, as future physicians we must dedicate ourselves to being lifelong learners of these intersectional topics since we are going to be taking care of individuals who are unique persons. ✨ • I am halfway through reading “How to be an Anti-Racist” by Ibram X. Kendi (which is the Common Book for my medical school class ) and I am loving every page of it. As I sit here on the eve of my last day of orientation, I’m again thinking about motherhood during medical school. I know it will add responsibility to my life and take away time, but I think it will also give me some distinct advantages. Here’s what I got: 1. I’m not afraid of poop or vomit. In fact, I’m almost a professional at cleaning up the messy stuff. 2. I have pediatric milestones burned into my memory. I will never forget at which age babies roll, babble, crawl, walk, etc. 3. I know what it means to be so sleep deprived you think you might die, but still be expected to take care of another life. But in all seriousness, I think the best advantage mothers have when going back to school is a different set of priorities that focus on family, relationships, and decompression. I'll be honest.. I didn't quite understand what everyone meant when they said "you're starting residency during a pandemic, be prepared".⁣⁣⁣⁣⁣ ⁣⁣⁣ Everyday, I walk into every patient's room with an N95, a surgical mask, and a face shield. I can't wait to take them off because I have a hard time taking deep breaths in them. But I can't.⁣⁣⁣⁣⁣ ⁣⁣⁣ Every time I leave a patient's room, I have so much anxiety about any possible way I could've contaminated myself. Did I sanitize enough times? Were my gloves on properly? Did my face shield move at all?⁣⁣⁣⁣ ⁣⁣⁣ Everyday I see nurses do their very best to provide the sickest patients with the care they need.⁣⁣⁣⁣⁣ ⁣⁣⁣ Every day I have a family member calling in asking what we can do to help their family.. whether its a new trial they read about or something else.⁣⁣⁣⁣⁣ ⁣⁣⁣ Everyday I see case managers/social workers do their very best to make sure that these patients who are "COVID survivors" have the proper care they need at home.⁣⁣⁣⁣⁣ ⁣⁣⁣ Everyday, I am more critical of myself than I thought I’d be. And most of this is because I know that I’m dealing with patients who can get very sick very fast, due to this virus. It’s like the adrenaline never leaves me. Even when I come home, I check my charts to make sure my patients are doing okay.⁣⁣⁣⁣ ⁣⁣⁣ I'm learning to not be hard on myself. I am consuming so much information every day, and the emotional baggage I take home every day is not light. I really care for the patients I'm in charge of. I thank the nurses who have been so patient with me. ⁣⁣⁣⁣⁣ ⁣⁣⁣ At the start of this rotation, you couldn’t tell the difference between me and a chicken with it’s head cut off.⁣⁣ And now, at the the almost halfway mark, I’m proud of the progress I’ve made. I’ve learned that to really progress, I had to be humble about my weaknesses, but not let my failures define my attitude. This really comes into play when you’re working in a fast paced setting. You just gotta keep moving forward. ⁣⁣⁣ ⁣⁣⁣ And so I tell you, keep moving forward.. for whatever it is in life. ❤️ There’s people to see, there’s people to save 👩🏻‍⚕️

TWO more weeks till medical school... This past week has been CRAAAAZY and exciting with my job with amazing opportunities and experiences I’d love to share. I’m fortunate to work underneath providers who not only push me to be better but allow me to question medicine, question reasoning and learn. I’ve never felt like I’ve belonged more in an opportunity then I do with my current job and I can’t thank my providers enough for everything they have given me. On Monday we had such a slow clinic day and I was working with one of the new providers for the first time. During clinic he turns to me and “goes want to go witness some surgery?” He takes me to the hospital and pulls me inside the endoscopy suite with some other providers and fellows and introduces me and says teach her everything and convince her to join us. It was the COOLEST experience. I got to sit on about 5 surgeries and even witness some rare cases that will be written up to be published. THEN, after surgery the other provider took me to the simulation lab we have in the hospital where medical students, residents and fellows can practice surgeries on “patients” and perfect their skills. He allowed me to mess around with a few of the endoscopes and so forth, (i did perforate a bowel in the process but hey I’m new🤷🏼‍♀️ ). It was so fun and my doctors encouraged me to come back whenever I have the time. It’s crazy to me that I am officially a medical student and I’m now about to start this insane journey of becoming a physician. Ever since I lost Henry this has been a dream for me. I’ve put my families life on hold because of it, I’ve put my husbands dreams on hold because of it, I’ve put us in so much debt (yay student loans ) because of it, I’ve missed out on events, holidays, social opportunities with friends just so I could prepare for this moment and now in two weeks this chapter will finally begin. I had my heart set on one specific field but now as I learn about all the crazy fields within medicine, I’m not sure where I’ll end up; but I do know I was made for this.

Recently an article that was published in July 2019 has been making the rounds on social media. A group of male vascular surgeons did a clinical study (by creating fake social media accounts to spy on applicants ) to determine how many vascular surgeons had participated in what they deemed as “inappropriate social media behavior”. Included in the definition of this phrase was females in bikinis. You can wear a bikini and still save lives.. when I first got accepted into medical school, one of the first things I did was go through my Instagram and review my posts. Being I live in AZ & it’s hot... ALWAYS, I have a lot of photos of me in bikinis. I looked at all my posts thinking “does this look unprofessional? Is anyone gonna take me seriously as a future doctor wearing this”. I fully believe in loving yourself, taking care of your body by working out & eating healthy. I am so confident in my body lately and the progress I have made with it. Isn’t that what wellness and medical professionals should exemplify to their patients? Shouldn’t we be promoting a healthier more positive lifestyle so no matter what size you are, you can learn to love yourself both physically, mentally and emotionally? Why is society making us believe seeing a women’s body is unprofessional? Showing the pride you have in yourself lays no correlation with how smart or professional you are. You are yourself before your medical career & you have a life outside of your medical career, we’d go insane if we did not. You should be able to wear whatever you want without being judged or not taken seriously. I don’t know HOW many times I have had this conversation with Steven about how I haven’t been taken seriously with my career choice to become a doctor, or been downgraded to a different career because the first thought is not that a female can be a doctor. At an appointment I had today I was talking about future plans & the possibility of having to eventually move out of state for residency and the persons first response was “oh, your husbands a doctor?” Like nope, that’s me, I’ll be the doctor in the family, thanks 🙋🏼‍♀️. ☀️WORK- LIFE BALANCE☀️ One of the most challenging aspects of medical school is establishing a good work life balance. It can be really hard to know when to stop working as there is always more studying that can be done📖 It’s so important for your mental health to have a life outside of medicine so here are some of my top tips🌟 ⠀⠀⠀⠀⠀⠀⠀⠀⠀ ✨Set Boundaries!⏰ I have a rule I don’t work after 9pm if I haven’t completed a task- it will just have to wait! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ ✨ Exercise 🏃🏻‍♀️ I make sure to get some form of physical activity in every single day, whether it’s a walk or a full on work out sesh, this really helps keep me in the right mindset. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ ✨ schedule activities🥂 I make a couple of fun plans with friends or family every week to look forward to! How do you rebound from a difficult experience? This path isn’t easy. Everything is not going to go according to plan and you won’t always be successful at everything. So how do you come back from a blow to keep moving? Recently, I’ve been experiencing this personally, as I read these ridiculous stream of papers that are being published. But in the past I’ve also failed important exams (scroll back ), had experiments fail, and had difficulties parenting. But failure is a part of life for most people. So there’s a few things I do to handle these situations. First, I don’t work for external validation. This is a difficult skill to gain but it’s easier to face hard situations with this thought in mind. It can’t always be about the reward, because the reward won’t always be present. When I do have these experiences, I give myself the chance to feel whatever I feel. So if you watch my story, you can typically see my response in real time (usually anger ). But I don’t stay there. Because that prevents me from moving on. Finally, I have to come up with a plan to move on. I can’t linger in the bad situation. So for the papers, I decided to do an objective review. For exams, I meet with our education counselor, I look into study methods, I figure out how to get better. But sitting in the disappointing situation is not progress. Situations like these are when discipline is more important than motivation. Motivation is not always present but if you focus on consistent behavior, you can keep moving.
