*Patellofemoral Pain* [PART 1/3] Patellofemoral pain (PFP ) is pain along the anterior knee or retropatellar region (pain located behind the knee cap ). This pain can be limiting in activities, such as walking, running, and working out. Learning the anatomy of the knee can help develop a comprehensive treatment plan. For instance, the quadricep tendon is attached superior to the patella and connects the quadricep muscles to the patella. It hurts when it does, sometimes less (when it just catches ) sometimes more (when it blocks and my knee won't bend at all ). . Class: it's the last students' presentations in my class, and I make the exam "live" #livesessionsdone . I'm still trying to reach my PTs, and their opinions on "dealing with symptoms vs. underlying cause" conundrum. Martin from CPM said he has no idea and that I should stay in the rehab clinic for 2 weeks. Martin from Kinisi didn't get back to me since mid-May at all 🙄. . It's a mental game now. I do hope my doc has a good plan but either I don't understand the plan or I didn't understand my doc how it should work #thelatterreally . Walkies. It's cold and windy today. I texted my rehab doc earlier as she told me to get back to her, not to "fall through the cracks," so I texted her about last developments over 6-7 weeks since we've last seen each other. . She replied in the best possible way 😍. That my ortho surgeon spoke with her, they deliberated together #miniconsilium , that my surgeon intends to write a case study on my odd case #yasss 🤞(she had no idea I suggested this to my doc; I hope it means he gave it some thought beyond the clinic ), that it looks like he'll need to loosen up the graft, that is seems too tight for a hypermobile me; and that it's logical that it affects movement of my patella as well as the catching/switching of my knee. . She writes that it seems surgery is my only option now, that I should find the courage and go for it, and that she keeps her fingers crossed for me that the surgery goes well and it's the last one!!! #sheisthebest 😍! . Runner’s knee: treatment and management Patellofemoral pain usually improves with a combination of load management and exercises.  As a physio, I am not a massive believer in the one size fits all approach, however based on the evidence and on my experience, there are common patterns of weaknesses which we usually see in this condition. Usually, the quads and glute muscles have become a bit sleepy so initially they need to be activated through the use of motor control exercises.  It is then necessary to train static and then dynamic movement control.  Once the correct movement patterns are in place, I will then teach a patient lower leg strengthening exercises and then plyometrics to increase power. In this post, I have shown you a few examples of these exercises at a few stages along the rehab process: 1️⃣ Muscle activation: Quads knee extension with theraband & Glutes side lying hip abduction. 2️⃣ Dynamic control: SL step up / down with band & rear elevated split squat. 3️⃣ Strengthening: Squat with theraband & Split squat I would recommend doing these exercises 8-15 reps (3-4 sets ) 3-4 / week. With all the standing exercises, ensure to focus on movement control and don’t allow your knee to move medially (inwards ) towards the other leg.  So try and keep your hip, knee and ankle in a straight line.  Performing the exercises in front of a mirror can be very helpful. Give them a go and let me know how you get on. Regarding load management – usually you will need to rest from running for a few days until the pain settles down. "Checking" if it still gets stuck #howaboutnow , trying to figure out where exactly… pondering whether I'll be able to show it on D-day #sure trying to narrow down the symptoms vs. underlying causes gap. I "play" with it like many times before… except today… it really hurts 😓. . As I get up to make coffee, as I walk around the flat. I try with the sleeve, without it… no change, both ways it's uncomfortable. My knee just doesn't move smoothly. It feels like every time I extend and lean on it, then take my weight off to relax and bend… it feels like it's about to dislocate 🤮. . Prepping late lunch in the kitchen, I stand almost exclusively on my rightie leftie is just there for balance. I actually ice in the afternoon. Trying to get the ache to go. It's indeed a proper annoying pain day. Haven't had one like this in months. . I need to work today, there's still so much to do! Like prepare the final exam for my class, then we go for a walkie... Knee catches with EVERY extension today so it's hard to do anything really. I'm aware of moving my hip more as I walk. . I can't wait to get rid of whatever is sticking in my knee. I'm still scared how it's going to end. A get s message from @aclhealing2019 about a procedure to loosen up the graft 🙏. She writes the same thing my surgeon was suggesting. I couldn't google it anywhere dr Mora suggested it doesn't exist. So it feels nice to know I'm not hallucinating. Walkies, maybe getting it moving will help? No. More ice? Runner’s knee: signs & symptoms When a runner comes to me with knee pain, the following factors will point me in the direction of diagnosing patellofemoral pain: . ➡️ Pain around the front or sides of the kneecap. ➡️ Increased pain when bending the knee. ➡️Worsening of pain going down stairs or walking and running down hills. Obviously, not everyone’s presentation fits this pattern and it will be the above signs and symptoms alongside their history, description of pain and then following a thorough physical assessment that usually then makes patellofemoral pain fairly easy to diagnose. I've tried to take a pic from the midline #pyjamasday It's overcast with drizzle. Knee is really creaky today. We stay in bed, on the couch all day, watching movies. I don't even bother to get out of my jammies today 😂🤷. . I just cook lunch, then we go back to watching films, I catch up on this journal... . but I can't sleep at night, guess I'm not tired enough. I keep going over the upcoming surgery in my head. Will it be arthroscopic or is it easier to cut the "retinaculum" from outside? What happens with the kneecap cartilage once the fat pad is debrided/removed at the top? Will it grind more, because the fat pad layer will be thinner? What's the recovery? Part 2 Patella femoral pain syndrome - What is it? Patella femoral pain syndrome (also just referred to as patella femoral pain PFP ) is pain in or around the patella femoral joint, it is not a specific diagnosis. (see part one for symptoms ) The patella femoral joint is where the knee cap connects with the thigh bone. The function of the patella is to act as a pulley with the quadriceps muscle to allow knee extension. Correct and balanced tension through the quads maintains proper tracking of the patellar, ie keeps it in the correct position. All 4 parts of the quadricep muscle attach onto the patella. Dysfunction in the quadriceps can pull the patella closer to the femur and this contact creates irritation and pain. Patella femoral pain syndrome causes a lot of controversy within the medical profession. The reason for this is that there is no actual damage found to the patella and femur where they come in to contact with each other with this condition. This can be very frustrating for the sufferer trying to get relief. There are 3 main possibilities, inflammation of the synovial membrane ( a membrane lining the inside of joints to keep them lubricated ), temporary damage to the patella bone or irritation of the femoral nerve. There are many reasons for this dysfunction and often treatment involves a thorough assessment of lower limb mechanics. Tomorrow we look at how to diagnose PFPS. Runner’s knee – The technical name for this is patellofemoral pain (PFP ). This injury accounts for 40% of reported running injurie. The pain originates from the patella (knee cap ) which sits in a groove over your tibia (shin bone ). As the knee bends and straightens when you run, the kneecap is supposed to track up and down in its groove. If there is muscle imbalance around the knee then this can cause the knee cap to slightly track out of it’s groove and lead to some irritation and then pain coming from the structures around it. Common causes for these muscle imbalances include: . ➡️ Altered recruitment of the quads muscles often including delayed firing of the VMO (most medial quads muscle ) . ➡️Weakness of the large lower limb muscles including quads, hamstrings, calfs, glutes. . ➡️Tightness of the anterior, posterior or lateral structures of the thigh including, gluteals, TFL, hip flexors, quads, hamstrings or calf muscles. All this week we are looking at Patello-Femoral Pain. Part 1 Do you have: - Pain around or behind the patella? - Pain when loading the knee - stairs, squats, running and jumping? - A crunching sensation when bending the knee? - Pain getting up from sitting or straightening the knee? - A tender patella when you press around it? These are all symptoms of the most common knee problem, especially in runners, patella femoral pain. PFP is a very poorly understood condition in terms of its pathology. Most cases are a result of overuse such as changes in intensity/duration and frequency of training. Hill workouts are a really good cause of this! He's aluminium unlike the boys' carbon frames (read: the heaviest ), and I wear the heaviest rubber which is nice and grippy in climbs and descents. Um, if surgery's the next plan, then so be it. He won't ever go against a doctor. He tries to "release me" (I don't think he's pulling hard enough 🤷 ). We're then basically working on core and hips and practice a phase of gait #basics . . When I ask why - he's like we're basically trying here to counter-compensate the compensations I do because I can't properly push-off my left leg when walking because I can't extend all the way and my hip has to move farther, which affects my pelvis tilt.... . When I ask him if he has a message to my surgeon, or what should I ask next time I see him... he doesn't have anything more 😑. He says he doesn't know. Normal day work and calls. I work on my revision notes for Friday. Help student services sort our  unregistered students, write info emails to end semester, summer courses ...and still need to write one nasty one for fall. If we'll have a drop in students, we'll need to drop numbers of courses and very likely also cut salaries. . Kuba arrives around 6 pm. I took a day off tomorrow. We're going biking. Even without reading all the ortho shit in the past two years - common sense - tells me: graft is in → shit starts to break down in my knee. We've tried to fix it → didn't work. We've tried to fix it again → didn't work. Isn't it time for the graft to go 🙄? . It's a busy day at the uni. A friend of mine from admin tells me he's going to run the business dept for a year as interim chair #mycondolences . Basically my position - different department. He gets along better with our boss 🤷. . And then he drops the bomb: he tells me my boss plans to have me do grad accreditation application by the end of June 🤬🤬. Insane, Impossible!! . Today is 19th of May; I need to revise with my class for finals, examine and grade, review theses for defence. Organize and double check student data for state exams, conclude the semester (sort grades and additional covid pass/fail paperwork ), update summer semester info, inform faculty about fall semester (likely cuts ), support the start of short summer semester on 1st June. All the while meeting on approval of internal policies, online teaching, budget for the next acad. year, and maintaining end of semester services for students (basically problematic students ). I should prep new marketing material too. And do pre-op labs & checks. State exams/thesis defence end on 12th June. 19th June is my surgery date and I'm taking 10 days off after NO MATTER WHAT. To compensate all those past working weekends. SO: OVER MY DEAD BODY 🤬! It's not physically possible to do it all #sešéfposralvkině ! We walk it off with Miwi. One year after the last one, almost to the day 🙄🙈. . It doesn't matter to my stupid knee if I slack a few days at home office or if I bike 93km on it over the weekend - IT STILL CATCHES, CRACKS (video ) AND LOCKS UP THE SAME 😭. Rehab did nothing, whatever is wrong is not a muscular but a mechanical problem 😑. . Nevertheless, I need to see him before; there's never enough time to chat on D-day and it's too late to plan. I want to talk it through with him once more. Why this plan, what happens to the graft? It's no joke anymore. It's been over 2 ½ years, 3 surgeries, I haven't done anything except rehab and bike 😑 (those few karate sessions don't count as I could barely move right ). No injury, no big fall, just complications… . All this time, all this work, and I have yet to have a good surgery outcome 🙏!!! Are we really addressing the underlying causes this time round or just treating symptoms again? I'm shitting bricks by now #seriously 💩💩. . I book a few nights at the rehab clinic after. So we're like: let's go easy, he's like #iwasdrinkingfor3daysstraight with my cousin 🙈. . But all's good. Legs are OK after yesterday's ride. We said we're going easy after all… Um, not with these two boys we're not 😝! Apparently, they chase each other ...and I chase after them 🙈. In the end, we're like 3-4 km faster on "average speed" than normal 🐒😂. 33 MTB kms 🙌. . Kubo sleeps over, we've ordered some curry and Miwi found some oooold biking videos. Which brings back ideas 😂. We want to do a long ride, so Ginger it is today (and bike tights 😝 ). Plan is to reach the ruins of castle Okoř (and the Únětice brewery of course 🍺 ). . Oooh, I looove spring bike rides 🤩. Strong sun but temp around 20°C are 👌 conditions. Knee on the other hand is not so happy after 20km and I can feel the kneecap gliding/grinding uneasy against its groove. So much so I have to put on sleeve after 35km… kneecap just wouldn't stop maltracking #bitch 😒.. . Ride is good, roads are, meh, stretches of good tarmac and then tarmac with waves and craters like the face of the moon 🌕🙈.. . My butt is properly "tenderised." Looks like more people had such good ideas, so it's quite full at Okoř. We catch some tan, eat bananas, attempt a few selfies 🤷 and move on. Brewery is nothing short of packed 🙈 #socialdistancingNOT . Beer's great tho 🍺! . Descent through Horoměřice is just 👌! Love it! End of semester/academic year exhaustion is setting in, I have a #4 surgery date still to confirm and yesterday's psych session was close to excruciating. It's better today, but I'm just soooo tired. Thank goodness(? ) for a busy day, at least it'll keep my mind occupied 🙏. . Testing online state exams - well that was a learning curve 😂🙈. Folks, read the instructions!! Afternoon is in the spirit of a Faculty senate meeting - 4 more hours of conference call on university policies, wages, covid affect Fall semester, management... It's juicy today 🙈🤬💩🥴 #theusual . Knee is disgusting during the day 🤮. As I work.. I dangle my legs and my kneecap totally catches and pulls. I don't know what to think about that "lateral release" surgery really. My knee didn't have a kneecap problem prior to recon 😑. On the other hand, god knows what could have developed in my knee these past 2.5 years 🙄. . Is it all that biking I do? Quads gain but there's an imbalance between quads and hammies now? My other knee clicks occasionally too. I thought it is from compensating the leftie. 💥Step Ups💥 - 🎯 In today's post, I'll show you several different ways to perform the step up, which, when done correctly, is a great single leg exercise - 🚨 Often, you'll see people performing step ups to a large box height, which looks impressive but can be deceiving as the person almost always hoists themselves up by pushing off of the back leg, decreasing the true work being done by the top "step up" leg - 💯 Instead, try lifting the back foot/toes off of the ground and KEEP THEM THERE, while also keeping the bottom knee locked out straight so that the majority of the work is being done by the "stepping up" leg (the leg you WANT to work! ) - 📌 You can also place the heel of the bottom foot on a weight plate (or something similar ), which keeps the toes off of the ground, providing another way to limit pushing off from the bottom leg - 🚨 You won't be able to use as high of a box, but the exercise will be a lot more effective - 🎥 Here I show 12 different variations of the step up: - 1 ) Bodyweight 2 ) Bodyweight - Toes off plate 3 ) Dumbbells - 2 Hands 4 ) Dumbbells - 2 Hands - Toes off plate 5 ) Dumbbell - Offset at side 6 ) Dumbbell - Offset at side - Toes off plate 7 ) Dumbbell - Offset Front Rack 8 ) Dumbbell - Offset Front Rack - Toes off plate 9 ) Barbell - High Bar on Back 10 ) Barbell - High Bar on Back - Toes off plate 11 ) Barbell - Front Rack 12 ) Barbell - Front Rack - Toes off plate - 💥 Also worth noting is that if you have knee discomfort with the step up, you can lean your trunk further forward and keep the knee further behind the toes - 💯 Conversely, if your knee feels fine and you want to load the quads more, you would want to keep the trunk more upright and let the knee travel more in front of the toes - ⁉️ Questions? ⭐️ MYTH/FACT: PATELLOFEMORAL ALIGNMENT ⭐️ . 👉 Having correct patella tracking and alignment is a concept that was heavily enforced in Physiotherapy . 👉 You had taping methodologies as a result and several exercises *cough* VMO, done to try correct it 😂 . 👉 Now if our goal is to establish good alignment then hypothetically those without patellofemoral pain should have good alignment as well. Or do they 🤔? . 👉 An interesting systematic review that looked at healthy participants and their patellofemoral alignment. Surprise surprise, the alignment was extremely variable 😮😮. . 👉 Now whilst we aren’t sure what “normal” alignment is, does that mean that there is no optimal alignment? . 👉 Perhaps not, it would make sense that there are efficient positions for our joints to work within, but that doesn’t necessarily mean it’ll cause pain. . 👉 But even then have a look at marathon runners and you’ll easily identify biomechanical insufficiencies. So is it really that efficient? . 👉 There are several factors that lead to pain and to pin it all on patellofemoral alignment when there are several pain free people with all sorts of “abnormal” or “normal??” alignment should be reconsidered. . 🤔 What are your thoughts! . . . . Did you know that patellofemoral pain syndrome has very little to do with your knee? #kneepain is the number two body issue that keeps people from physical activity and hijacks athletic performance (number one is #lowbackpain and number three is #shoulderpain ). If you’re patella isn’t aligned properly along the groove in the femur and the tibia, you’ll wobble when you squat, jumping will cripple you and anything faster than a shuffle is going to be extremely uncomfortable. . 🙏PATELLOFEMORAL PAIN HACK🙏 . This video is designed to help you work through #patellofemoralpain (PFPS ) and over time, eliminate the tissue extensibility problem, and reprogram how you move. The first thing you need to understand is that the problem is because of four primary issues: 1. External rotation weakness 2. Posterior chain extensibility loss 3. Tissue adhesion to the #ITB 4. #CNS motor control dysfunction . First, test your might: Get into a simple side plank, if you can’t maintain it for 60-90 seconds each side, you’ve got an #abduction fault. FAIL. . In order for the patella to ‘grease the groove’ the #femur and the #tibia need to be in parallel, so when your knee collapses into valgus, the mechanics are destroyed. Here’s what’s happening: 1. The biceps femoris is tight, it acts like a bowstring and collapses the knee. 2. The vastus lateralis loads the ITB and forces knee collapse. 3. The gluteus medius either doesn’t fire 🔥 or is weak, allowing the femur to roll inward. 4. Every movement you make reinforces this Mickey Mouse movement pattern. . PATELLOFEMORAL PAIN — Patellofemoral pain is a fairly non-specific diagnosis given to individuals experiencing pain at the front of the knee that is commonly attributed to weak glutes, tight quads, poor movement patterns or a combination of the three. — While it is often helpful, and recommended, to strengthen the hip and knee musculature (usually the glutes and quads ) for individuals experiencing patellofemoral pain, it is only examining the diagnosis from a biological lens. — Rather than focusing on the patellofemoral joint in pain, it’s important to focus on the person with patellofemoral pain as it can be associated with “higher levels of anxiety, depression, catastrophization, and fear of movement”. — Now, we shouldn’t compartmentalize our treatment or rehab into biological, psychological, or social constructs. Instead, we should focus on how the experience of pain affects each person. — Modify the contributing factors that are modifiable. Strengthen the glutes and quads. Manage the load. Don’t INDUCE fear or anxiety by providing a nocebic narrative about how their tight/weak/broken _____ is the cause of all their problems. And consider how this pain is affecting their day to day life. Maybe they can’t play sports or walk their dog with their significant other. This is important information to tease out to help guide goals for rehab.

Here’s a member requested repeat 💯💪 Did you know that patellofemoral pain syndrome has very little to do with your knee? #kneepain is the number two body issue that keeps people from physical activity and hijacks athletic performance (number one is #lowbackpain and number three is #shoulderpain ). If you’re patella isn’t aligned properly along the groove in the femur and the tibia, you’ll wobble when you squat, jumping will cripple you and anything faster than a shuffle is going to be extremely uncomfortable. . 🙏PATELLOFEMORAL PAIN HACK🙏 . This video is designed to help you work through #patellofemoralpain (PFPS ) and over time, eliminate the tissue extensibility problem, and reprogram how you move. The first thing you need to understand is that the problem is because of four primary issues: 1. External rotation weakness 2. Posterior chain extensibility loss 3. Tissue adhesion to the #ITB 4. #CNS motor control dysfunction . First, test your might: Get into a simple side plank, if you can’t maintain it for 60-90 seconds each side, you’ve got an #abduction fault. FAIL. . In order for the patella to ‘grease the groove’ the #femur and the #tibia need to be in parallel, so when your knee collapses into valgus, the mechanics are destroyed. Here’s what’s happening: 1. The biceps femoris is tight, it acts like a bowstring and collapses the knee. 2. The vastus lateralis loads the ITB and forces knee collapse. 3. The gluteus medius either doesn’t fire 🔥 or is weak, allowing the femur to roll inward. 4. Every movement you make reinforces this Mickey Mouse movement pattern. .
