The latest life expectancy report from the Census and Registrar General’s office highlights the importance of the first year after birth. It shows that the life expectancy of a person increases by almost two years if they survive this period, indicating just how much infant mortality is impacting the country. Some days I truly wonder when the fear of birth began. I sit back and I ask myself, why are people so scared of something so beautiful? When I ask parents what research has been done into where they choose to give birth, the automatic response is at the hospital, and yet many do not even know of these staggering statistics. The Bronx has the highest rate of maternal/perinatal and infant mortality in all of New York State. My dear brothers and sisters that identify as BIPOC, our birthing folx are dying at more than 12x’s the rate of their yt counterparts, and that was BEFORE the COVID-19 #Scamdemic . These hospitals have triple and quadruple the rate that they are recommended to abide by. Western allopathic medicine has its place, but birth is not a medical event. Boosting breastfeeding support is important for black women and their children because it can improve their own health outcomes and mortality, which lag behind other groups in the U.S. Black women are three to four times more likely to die as a result of pregnancy, compared to white women. I keep thinking about the way #GeorgeFloyd called for his mom. I keep thinking about the mothers of Breonna Taylor, Ahmaud Arbery, Sandra Bland, Eric Garner, Tamir Rice, Atatiana Jefferson, Alton Sterling and so many others. Did they know about what their babies went through in those last moments? I think about all the black mothers who die at 4 x the rate of White women in childbirth, making America the nation with highest rates of #maternalmortality in the developed world. This issue is intersectional. The defense of black and brown bodies and lives means a commitment to the #reproductive justice framework. As a woman of color and mother of two who has a lot of privilege and access, I can’t even fathom the thought of having my own choices over my body and my kids taken away. On any given day, I may perform: ⁣ -A vaginal delivery ⁣ -A cesarean delivery ⁣ -A cesarean hysterectomy (to save a bleeding mom )⁣ -A cerclage (for mom’s with short cervix )⁣ -A tubal ligation (for mom’s that no longer desire to have additional kids ) ⁣ -A dilation and evacuation (for mom’s that may have had a miscarriage ) ⁣ ⁣ All of this.....in addition to performing fetal ultrasounds, doing fetal interventions to help determine if the baby has a genetic disorder, and seeing my high risk pregnant moms because they need our special attention. ⁣ ⁣ All of these skills can be performed by your Maternal Fetal Medicine specialist or “high risk doctor”. ⁣ ⁣ We train for a total of 11 years (4 years of medical school, 4 years of residency, and 3 years of fellowship ), sometimes more just so we can give the utmost and comprehensive care to our patients. ⁣ Let’s continue to tackle this fight together 💪🏾 ⁣ #highriskpregnancy #maternalmortality #mfm #maternalfetalmedicine #expecting #postpartum #doulasmatter #doula#doulasupport #birthdoula #obgyn #womenofaka

Today, I thought I’d reflect on how far I’ve come in my medical career journey. For those that don’t know, I’m not only an OBGYN but I’m training as a specialist in Maternal Fetal Medicine. ⁣ ⁣ It’s a special group of OBGYNs that take care of women with complicated pregnancies. These women have complex problems that result in high risk but high rewarding pregnancies. ⁣ ⁣ I love giving “high-fives”and singing Happy Birthday during deliveries knowing we just made it through 9 long, hard months of one of the hardest experiences in their life. ⁣ ⁣ If there’s anything I’ve learned about these women...it’s that they will do anything for their baby and they deserve a provider that will do anything for them. 💪🏾💪🏾 ⁣ ⁣ #mfm #highriskpregnancy #maternalfetalmedicine #obgyn #melanindoctorsrock #maternalmortality #expecting #reseaudocteur

𝖠𝗅𝗂'𝗌 𝖻𝖺𝖻𝗒 𝗌𝗁𝗈𝗐𝖾𝗋, 𝗈𝗇𝖾 𝗒𝖾𝖺𝗋 𝖺𝗀𝗈 𝗍𝗈𝖽𝖺𝗒 💗💗. . . . . It’s hard to talk about, but it’s so important to advocate for her and raise awareness. Amniotic Fluid Embolism is a rare, but often fatal, complication of pregnancy and childbirth. There is very little research into this condition, and currently no known way to predict or prevent it. . . . . The United States has the HIGHEST maternal mortality rate in the developed world. We also have the highest racial disparity rate, with Black women being 3-4X more likely to die in childbirth than white women (regardless of socioeconomic differences ). . . . . All of us mothers need to advocate for more research, better education and improved policies so that EVERY mama has a safe delivery. . . . . . The newly-formed @alisonrosefoundation will work to fund research into Amniotic Fluid Embolism, to promote awareness, and to provide support for mothers, babies, and families who are affected by severe complications of pregnancy and childbirth. I am honored to be a board member, and am determined to honor my best friend by helping other families, advocating for change, and working to #endAFE . . . . . One initial, easy, and effective way that you can help is to #donateblood . AFE, along with other severe complications of childbirth such as placental abruption and hemorrhage, requires blood transfusions as part of the emergency treatment. In some cases, women who have survived AFE have required around ONE HUNDRED units of blood in their treatment. Share your birth story to help fight racism in maternal care. . A Black-led project ( @dr .rrh 🙌🏿🙌🏽🙌🏻 ) is collecting pregnancy and birthing stories from Black moms and birthing people to train prenatal providers across California about the dangers of implicit bias and disrespectful care. The trainings will also focus on reproductive justice in accordance with the training requirements outlined in the California Dignity in Pregnancy and Childbirth Act (Senate Bill 464 ) which went into effect in January 2020. #Pregnancy outcomes for Black women I’ll never forget the day I gave birth to my child.  I was extremely cautious during my pregnancy and had vigilant monitoring of my fibroids.  I felt anxious during the pregnancy because I knew that Black women have the highest maternal mortality rates during pregnancy or within a year of pregnancy. Black females are more likely to suffer from pregnancy related complications like preeclampsia/eclampsia, postpartum hemorrhage from fibroids, nonelective C-sections or cardiomyopathy (enlarged heart ). As a physician, I knew the statistics for Black women and now as a soon to be mom, those statistics were screaming in my head. I just prayed that my child would be healthy and that I would make it through a C-section. I was terrified! This issue of implicit bias which exists in healthcare is especially worrisome in women’s gynecological and maternal health.  If the patient feels that her OB/GYN displays bias, then she may feel that her physician is not worried about her health, the outcome of her pregnancy or her baby’s survival. That female may not feel comfortable or be inclined to ask questions pertaining to how to ensure a healthy pregnancy and post pregnancy care. She also may not be aware of the necessary follow up visits for ultrasounds, blood pressure monitoring, urinalysis checks, and blood sugar/ glucose testing. Black females may feel more empowered to take more control of their health through education about pregnancy complications If they have a healthcare provider that looks like them or has completed cultural competency training.  They may be more likely to commit to healthier lifestyles to mitigate risk for conditions such as Pregnancy induced hypertension, Gestational Diabetes and Pre-eclampsia/Eclampsia. They may also feel that the OBGYN can possibly better understand the barriers, mistrust or suboptimal care that these female patients may have faced trying to achieve equality in their maternal healthcare experience. Minority females can be their own advocates and TAKE ACTION! Stay tune for Part II on how minority females can improve the outcomes for their pregnancy!
