Let’s talk about this in the context of low back pain (LBP ). Available evidence tells us that LBP is a complex health condition influenced by multitude of biological, psychological, social and environmental factors, which all interact to create the experience of pain (Zaina et al., 2020 ). These domains of the biopsychosocial model cannot be separated neither can the person be separated from its environment (Stillwell & Harman 2019 ). Thus, we can’t talk about pain being only anatomical (e.g. particular muslce, joint, disc ) or psychological etc. . LBP is more nuanced and multifaceted, and we can’t attribute that to a singular cause as we often unfortunately do e.g. a posture, lifting technique, tight or weak muscles or “stiff” joints. That being said, our efforts to manage the symptoms should also be multifaceted. For me it would not make sense to manage LBP purely on interventions aimed towards the structure of the spine, like manual therapy, if the person is reporting high levels of anxiety, worry, stress, low self-efficacy and lifestyle factors such as physical inactivity. . Passive treatments can be used, however it is important to note that pain is not the result of lack of massage, or spinal manipulation, or stretching. There is not going to be a detrimental effect of not using these modalities. On the other hand, if you’re sleep deprived, stressed, physically inactive, cope passively and have low self-efficacy, you are at a higher risk of developing persistent LBP. . So instead of “correcting posture” or “loosening tight muscles” (OR actively strengthen ”weak” muscles ), lets look at the big picture ”and add more weight” to that. Listen and help make sense of pain, focus on language that empowers rather than finds faults to correct, educate and advice, guide self-management, manage lifestyle factors and focus on doing more of the things that the person experiencing pain enjoys and wants to do. . Lets provide more effective and high value evidence based care for individual with LBP (or other MSK ) in 2020.

Mercoledì 8 aprile, in quanto operatrice sanitaria riaprirò i battenti. Naturalmente ci è cambiata la vita, e dobbiamo abilmente adattarci per non soccombere. Pensiamo alla tartarughina che lemme lemme ha resistito fino ai nostri giorni, e al dinosauro volone che si è estinto. Applichiamo la legge di Darwin in ogni contesto della nostra vita: ADATTIAMOCI. Per ciò, ho dovuto aggiungere alcune regole, fino all’avvento di una cura reale o di un vaccino. •Non esisterà più la sala d’aspetto e l’assembramento di persone fuori dalla porta. •sarà sospeso l’eccesso libero nei giorni di lunedì mercoledì e venerdì, fino a data da destinarsi. • le sedute avranno una durata compresa tra i 15 e i 20 minuti per contenere l’eventuale possibilità di contagio. •non potrò prendere in carico chi si presenta senza adeguata mascherina. No foulard, bandane e straccetti in bocca. Non saranno fatte eccezioni. •Di concerto non prenderò in carico chi si presenta con attacchi di tosse secca o grassa, siano pur essi dovuti a tabagismo o a reflusso gastrico, non sta a me fare diagnosi. •Gli ingressi saranno contingentati, di modo che le persone non si incrocino e di modo di avere tempo necessario per sanificare in modo maniacale lettino, maniglie, sedie, porte e me medesima. •lavorerò munita di guanti, visiera da dentista , mascherina , cuffia per capelli. QL-"ish" BACK PAIN & SCAPEGOAT MUSCLES . As prevalent as low back pain is, we can't really pin down one tissue or pathology as a main cause. In fact, it's estimated that upwards of 90% of back pain is nonspecific, meaning it isn't attributable to any one thing we can identify. . With that in mind, it's really common for people to present with low back pain primarily on one side. . And due to its location, it is often attributed to the Quadratus Lumborum muscle, or QL for short. . The QL is what I would label a "scapegoat muscle". . It gets thrown into the same category as the hip flexors, hamstrings, pecs, and SCMs. These muscles are often described as culprits in people's pain. Sometimes they're weak. Sometimes they're tight. Sometimes they're tight and weak. 😐 It just depends on who you talk to. . Yet we don't have any plausible mechanisms as to how these muscles could lead to pain. . Pain is more complex than "tight this" or "weak that". And we shouldn't reduce it down to that. . Pain is a better representation of the current sensitivity of an area than it is the state of a tissue. . So with that in mind, realize that we don't have to necessarily change a specific tissue to change the pain. . And while exercises like the video above may look like they are for a specific muscle (QL ), they really are just a way to direct some load to the area of sensitivity. . And they can very likely help with the sensitivity there. Maybe because we strengthen something. Maybe because we improve some endurance. Maybe because we alter the current sensation with a new one. Or maybe because we're creating a positive expectation for improvement by working the area. . It could all be a factor and that's the complexity and beauty of the human body. . But given we rule out the serious stuff and acute tissue damage, likely the best solution is not to focus in on a scapegoat tissue that must be changed as this can leave us with a "I need to be fixed mentality", but rather view it from a sensitivity standpoint, scale our activities to our current tolerance, and work on gradually improving that tolerance to return back to normal function. . #Prehab101

Low Back Mobility💫 . Sometimes things are real tight and all you feel like doing is anything that DOESN’T require moving, but actually- in those situations- some movement is gonna be your best friend. . And if you’re not experiencing discomfort this routine might be a great first phase warm up. . Here are some ideas to get that motion rolling. 👉🏻If you’re doing this because you’re sore- you’d likely want to keep your back in contact with the ground and just move from the legs while probs taking it slow. 👉🏻As more of a warm up you can add some extra ROM and likely speed- you’ll see i’m all over the place and that’s because the extra ROM felt amazzzzzing. . I recently wrote a post for @sweat_minnesota on how I’m taking care of my mental health, right now. To be honest, it’s not that much different than my typical practices, but I definitely found myself leaning on them a bit more during the first few weeks of uncertainty. To read the full version, check out the link in my bio • • Let’s be honest. This fucking sucks (and I’m not even an extrovert ). People are losing jobs. Businesses are losing money. The health of the world is suffering. And while some people are suffering physically, almost everyone is suffering mentally. We often forget that our bodies and our minds are one in the same. Not just connected, but synonymous. When one suffers, so does the other. That’s why, during this period of physical isolation, I urge you to protect your mind as you would your body. Here’s some things to focus on (more in depth explanations & action steps in article ) • • • Gratitude: even in the toughest situations, there’s numerous things to be grateful for. Reflect on at least 5 things each day • Breath: altering breath will alter the state of our body. During a time of stress, it is important to focus on slow, controlled diaphragmatic breathing. This can have an immediate calming effect •Media Diet: This is a big one. I highly suggest you limit your time on social media & news sites. There is a lot of information, both accurate and inaccurate, flying around which can be stressful to consume •Acknowledging & Sharing Emotions: You might feel like you're on an emotional rollercoaster right now, and that’s okay. Acknowledge what you’re feeling •Exercise: Movement keeps the mind happy. It releases feel good chemicals such as serotonin and dopamine which help to improve mood and sense of well-being . . . I also like to think about what I’m being taught, right now. What am I learning about myself? I’m also thoroughly enjoying spending more time with my little fam & more time outside . . @sassypantsphoto 📸
